First Appointments: If this is your first appointment, you will receive an email with intake paperwork and a credit card authorization form. Both must be signed and returned before your first appointment. Most folks like to talk to me before making an appointment, so it you haven’t please feel free to call me.
Once you make the appointment you will receive a confirmation email. If you don’t, please contact me.
You will receive an email within 24 hours once your appointment has been approved.
